On an Armenian form the fields are short, and the name goes last name first: Սմիրնովա Օլգա. The table shows the fields.
To say when you were born, use the form's set phrase: Ծնվել եմ 1995 թվականին — “I was born in 1995”. The year is in figures. The place takes the familiar ending -ում: Ծնվել եմ Ամերիկայում.
Ordinal numbers (first, second, third …) come before the noun and don't change: հինգերորդ հարկ — the fifth floor, հինգերորդ հարկում — on the fifth floor.

• երկրորդ, երրորդ, չորրորդ — end in -րորդ
• from five on — the number + -երորդ: հինգերորդ, վեցերորդ; ինը and տասը swap the final -ը for -ն — իններորդ, տասներորդ
